I am aware of my own health / physical condition and know that my participation in this sporting event may be injurious to my health, and I am voluntarily participating in this sporting event. Having such knowledge, I release Jefferson Prep and Associates from liability for accidental injury or illness that I may incur as a result of participating in this sporting event. I assume all risks connected with and consent to participate in this sporting event.
